• Knowledge of CMS Skilled Nursing Facility regulations.
• Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles.

Responsibilities

• Conduct medical exams, take histories, and diagnose common illnesses.
• Order and interpret lab tests and imaging as needed.
• Develop and implement individualized treatment plans.
• Prescribe medications and monitor patient response.
• Educate patients and families about healthy habits and wellness strategies.
• Stay current with changes in medical policies, procedures, and regulations.

Responsibilities:

Compensation

• Competitive Total Compensation: $125,000-$150,000
• Performance Incentive Program through Hero's Provider Incentive Compensation Plan
