Primary Care Physician opportunity in New Albany, IN
Wonderful opportunity to work as a lead in our team-based care environment. We are a value-based care provider focused on quality of care for the patients we serve. Our care team consists of doctors, advanced practice professionals, Pharm D, care coach nurses, MAs, behavioral health specialists, quality-based coders, referral coordinators, and more.
Responsibilities:
- Evaluate and treat center patients in accordance with standards of care.
- Monitor the level of medical care and quality for patients, utilizing available data and chart reviews.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
- Complete all medical record documentation promptly, working with a quality-based coder to optimize coding specificity.
- Adhere to policies and protocols defined by Clinical Leadership.
- Meet with RMD to review quality of care, outcome data, policies, procedures, and records issues.
- Participate in growth opportunities for new or existing services within the Center.
- Participate in the local primary care on-call program as needed.
- Ensure compliance with licensing, certification, and accreditation standards.
- Spend 100% of your time clinically focused on direct patient care, including patient-facing activities and related administrative tasks such as charting and meetings.
Preferred Qualifications:
- Board Certification or eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active and unrestricted DEA license.
- Medicare Provider Number.
- Minimum of two to five years of directly applicable experience preferred.
- Experience managing a Medicare Advantage panel with an understanding of best practices in a coordinated, value-based care environment.
- Knowledge of Medicare guidelines and coverage.
- Bilingual skills are a plus.
- Knowledge of HEDIS quality indicators.
