Primary Care Physician - Chicago, IL

COMPENSATION: Salary $250k + Incentive Bonus + Signing Bonus up to $50k

To achieve our vision and fulfill our mission, we seek dedicated primary care providers and clinical leaders who are looking for purpose and personal growth. Transportation is provided to patients at no charge.

Role:

1. Compensation: Base $250k-$270k
2. Bonus: Up to $20k in the first year, increasing as a partner
3. Partnership: No buy-in; typically within 12-18 months; 65% achieve partnership after 1 year
4. Patient Load: 18 patients per day
5. Hours: Monday - Friday, 7:30 am - 5:00 pm (last patient scheduled at 4:15 pm)
6. Work/Life Balance: Emphasized
7. Patient Panel: Max 450 patients
8. Administrative Time: Included in schedule
9. No nights or weekends
10. Practice Setting: 100% outpatient
11. Support Team: Dedicated MA, Scheduler, Referral Specialist
12. Patient Demographics: Seniors with 3-5 chronic conditions
13. Care Model: Value-Based Care
