Job summary

DayToDay Health seeks an Outpatient Primary Care Physician in Florida to serve Medicare Advantage senior patients within a value-based care setting. You will deliver comprehensive, patient-centered outpatient care and drive prevention and good health outcomes.

You will work with a dedicated team in state-of-the-art facilities, providing proactive outreach, same-day urgent appointments, and transitional care to ensure smooth patient journeys and high-quality care.

Qualifications

• MD or DO with preferred family/internal/geriatric medicine.
• Active Florida medical license, DEA license, and CPR (BLS/ACLS/PALS).
• Proficient with EMR, G-Suite, and database programs.

Job description

Role: Outpatient Primary Care Physician in Florida, serving Medicare Advantage senior patients in a value-based care setting.

Responsibilities Include

• Providing comprehensive outpatient care with a focus on prevention and maintenance.
• Managing smaller patient panels, typically seeing 12-15 patients daily.
• Encouraging preventive visits, proactive outreach, same-day urgent appointments, and transitional care.
• Utilizing electronic medical records, ICD-10 coding, and STAR measures to ensure quality outcomes.
• Delivering patient-centered service in state-of-the-art facilities with a dedicated team.

Qualifications And Skills

• Medical Doctorate (MD) or Doctor of Osteopathic Medicine (DO), preferably in Family, Internal, or Geriatric Medicine.
• Florida medical license, DEA license, and current CPR certification (BLS, ACLS, PALS).
• Proficiency in EMR systems, G-Suite, and database programs.
• Effective communication, problem-solving, organizational, and task prioritization skills.
• Ability to complete a pre-placement Tuberculosis test.
