Family Practice - Geriatrics Physician at CenterWell Senior Primary Care summary :

The Family Practice - Geriatrics Physician at Conviva Senior Care Centers is responsible for providing senior-focused primary care, addressing patient needs, and understanding the social determinants of health impacting patient outcomes. This role involves direct patient care with a typical schedule of Monday to Friday, emphasizing longer appointments and a collaborative care team approach. Ideal candidates are board certified in Family, Internal, or Geriatric Medicine, with experience in managed care, particularly within Medicare Advantage.

Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a Family, Internal, or Geriatric Medicine Physician for our clinic in Corpus Christi, TX.

You have the opportunity to take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are impacting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a multidisciplinary approach.

A day in the life of a Conviva physician

• Monday-Friday 8:00 am - 5:00 pm; no weekends, and 8 paid holidays off
• Flexible schedule available, if needed
• Join an experienced team with two Advanced Practitioners and a Care Team
• See 12-15 patients per day. Opportunity to spend more time with patients
• Longer authorized appointments (45 minutes)
• Acts as a lead and active participant and key source of medical expertise with the Care Team through daily huddles
• Spend 100% of your time clinically focused on direct patient care, including patient-facing time and related administrative tasks (charting, meetings, etc.)

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine
• Valid, unrestricted license in the state of desired employment (or willingness to obtain)
• Current, unrestricted DEA license

Preferred Qualifications :

• Minimum of two to five years of directly applicable experience preferred
• Board Certified / Board Eligible in Family, Internal, or Geriatric Medicine
• Experience managing a Medicare Advantage panel with an understanding of best practices in a coordinated, value-based care environment
• Knowledge of Medicare guidelines and coverage
• Knowledge of HEDIS quality indicators
• Spanish language ability
