1. MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required.
2. Current, active MD licensure in the state of employment is required.
3. A minimum of 1-year clinical experience in geriatric, adult, or family practice setting is preferred.
4. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; Board eligibility is required.
5. Once board certified, the PCP will maintain board certification in their terminal specialty through necessary MOC, CME, and/or retaking board exams as required.
