This full-time role involves working collaboratively in a primary care team, engaging in both outpatient and facility-based care, and offers mentorship and opportunities for education and research.

The Division of Community Internal Medicine, Section of Senior Services, is seeking a general internist or family medicine physician to provide clinical/outpatient services for high-risk older adults in subacute and long-term skilled nursing facilities. The full-time positions are located in the primary care clinics on our Austin and Albert Lea campuses, with care provided in skilled nursing facilities around these communities.
The physician will join a primary care team of physicians and advanced practice providers, working collaboratively to deliver holistic care, including:
Ideal candidates will have experience in primary care, geriatrics, and palliative care. Travel within Southeast Minnesota will be required, potentially including shared time in Rochester. The role may also offer opportunities to serve as a medical director in the facilities.
