Job summary

CBHA is seeking a Family Planning Coordinator to ensure high-quality, well-coordinated patient care across clinic sites in accordance with Title X guidelines. You will work with the VP of Programs and medical team to optimize patient care, supplies, documentation, and staff support, including monthly site rounds and annual outreach activities.

The role requires collaboration, training duties, HIPAA compliance, and bilingual English/Spanish communication with patients.

Qualifications

  • Bachelor’s degree or higher in a health-related field; public health preferred.
  • Experience training others.
  • HIPAA compliance knowledge.

Responsibilities

  • Implement family planning program activities per Title X requirements and CBHA guidelines.
  • Coordinate reportable STI follow up with medical providers, patients, and health department.
  • Purchase and order program supplies and education materials for patients and staff.
  • Provide technical assistance and training to medical care team on family planning services.
  • Oversee and complete documentation per Title X guidelines.
  • Complete project reporting with timely, quality deliverables.
  • Maintain excellent patient service relationships and represent CBHA professionally.
  • Coordinate and participate in outreach events to promote services.
  • Uphold confidentiality and WE CARE Standards of Behavior.

Overview

The Family Planning Coordinator (FP Coordinator) will carry out responsibilities which insure high quality, well-coordinated patient care for the clinic according to Title X Family Planning guidelines and the mission of CBHA. The FP Coordinator will work closely with the Vice President of Programs and the medical care team to insure that activities related to patient care, supplies, documentation, and the support for staff are optimized for efficient and compliant delivery of high quality patient care and family planning services. The FP Coordinator will round with each clinic site at least once per month to ensure consistent practices and opportunities for training and will participate in outreach activities throughout the year.

Responsibilities
  • Responsible for implementing family planning program activities as outlined by Title X Family Planning Program requirements and CBHA guidelines.
  • Coordinate reportable STI follow up with medical providers, patients, and health department.
  • Responsible for purchasing and ordering program supplies and education materials to support the needs of patients and program staff.
  • Provide technical assistance and training to members of the medical care team regarding family planning program and services.
  • Oversee and complete documentation of required information per Title X guidelines.
  • Responsible for the completion of all project reporting, including timely, quality, and concise reports necessary for project deliverables and to evaluate project outcomes.
  • Maintain excellent patient service relationships and represent CBHA professionally in all work related interactions with patients, the medical care team, community partners, and Title X colleagues.
  • Responsible for coordinating and participating in outreach events to promote and educate on family planning services and resources.
  • Promotes a positive work culture and upholds clinic policies and procedures, including the WE CARE Standards of Behavior.
  • Must uphold confidentiality at all times in regards to CBHA activities, reports, financials, patient health information and other proprietary information specific to CBHA.
  • Other duties as assigned.
Qualifications

Professional and Technical Knowledge:

  • Completion of a bachelor’s degree or higher in the field with relevance to health care. Candidates with relevant work experience or advanced education in public health, health administration, or another relevant field will be considered with preference.
  • Experience in training and developing the skills of others.
  • Knowledgeable and maintains compliance with recognized HIPAA regulations.

Technical Skills:

  • Ability to prepare correspondence and reports utilizing Microsoft software and/or e-mail.
  • Ability to create tables and simple displays of information into spreadsheet software.
  • Ability to enter and manage data into a centralized electronic health records.
  • Ability to set up and coordinate phone conferences utilizing technology and conference room systems.

Communications Skills:

  • Possesses the ability to successfully form, foster and improve relationships with members of a culture different from one's own.
  • Possesses ability to meet with a variety of people, both formally and informally, and to communicate in a manner that the receiving party will clearly understand.
  • Job duties require the effective communication of information in written (including electronic) correspondence.
  • Bilingual in English and Spanish to speak with patients.
