Job summary

Scarborough Health Network (SHN) is seeking a family physician to join the Interprofessional Primary Care Team and Primary Care Teaching Clinic. The role emphasizes medical education, supervision of residents and students, and maintaining a faculty appointment with the University of Toronto.

You will provide comprehensive family medicine services, work within a collaborative team, and contribute to quality improvement and patient-centred care in a university-affiliated setting.

Qualifications

• Current registration and a member in good standing with the College of Physicians and Surgeons
• CCFP certification is preferred
• Strong knowledge of primary health care and chronic disease management in a community setting
• 2–3 years recent clinical work experience as a Physician with community experience preferred
• Excellent verbal and written communication skills

Responsibilities

• Provide routine and preventative healthcare services to a diverse patient panel
• Develop personalized treatment plans including medication management and referrals
• Manage chronic conditions such as diabetes and hypertension
• Maintain accurate electronic health records to support patient care
• Collaborate with inter-professional team to coordinate care and transitions
• Support education and supervision of residents and medical students
• Engage in health promotion and disease prevention activities
• Relate with hospital, specialists and community services to optimize care
