Job summary

PATHS Inc. is seeking a dedicated physician to provide primary care across age ranges within a Patient Centered Medical Home model. The role includes clinical leadership and supervision of midlevel providers, contributing to high-quality care delivery in a team-based setting.

The physician will manage direct patient care, document comprehensively in the EHR, and participate in on-call shifts while maintaining confidentiality and adherence to policies and CME requirements.

Qualifications

• Graduation from an accredited medical school.
• Current Virginia medical license.
• DEA certificate.
• Board certification is preferred.
• CPR certification is required.
• Credentialed per Community Health Center policies for Medicare/Medicaid billing.
• Supervises midlevel providers and reviews charts to ensure quality of care.

Responsibilities

• Provides and manages direct patient care including exams, evaluations, diagnoses and treatment.
• Documents patient history, systems review, and plan within the EMR.
• Issue medical orders for refills, tests, and referrals per policy.
• Review test results and notify patients per policy.
• Provide education in disease management and prevention.
• Complete required documentation in the EMR for insurance and reimbursement.
• Participate in on-call schedule.
• Maintain patient confidentiality at all times.
• See 21–25 patients per day on average.
• Establish protocols with staff and providers in the marketplace.
• Review mid-level provider medical records per policy.
• Follow agency policies, safety, and infection control standards.
• Engage in health promotion, education, and prevention programs.
• Maintain licensure and CME requirements.
• Attend staff and provider meetings.
• Performs miscellaneous job-related duties; supports PATHS mission.

Job description

Under the general supervision of the Chief Medical Officer, provides primary care to patients ranging in age from birth to geriatric. Works as part of a care team that practices evidenced based medicine in a Patient Centered Medical Home model of care. Provides clinical leadership and supervision of midlevel providers

Education and/or experience

• Graduation from an accredited school of medicine
• Hold current license to practice in state of Virginia
• Current DEA certificate
• Board Certification preferred
• CPR Certified
• Credentialed per Community Health Center Policies and Procedures for billing services under Medicare/Medicaid
• Supervises assigned midlevel providers in the conduct of clinical care and delegates' authority when and where needed.
• Reviews assigned midlevel provider's patient charts to check for quality of care.

Clinical

• Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Obtains and documents, or causes to be obtained and documented; appropriate chief complaint, past medical, family and social history, review of systems, examination, medications and allergies, assessment and plan for each patient within the electronic medical record.
• Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals within electronic medical record as per policy.
• Reviews ancillary test results and coordinates notification to the patient as per policy.
• Provides education in disease management and prevention.
• Completion of appropriate chart documentation within EHR to comply with insurance and reimbursement guidelines
• Completion of all required documentation within EHR within a timely manner as per policy.
• Participates in on-call schedule.
• Maintains patient confidentiality at all times.
• Will see on average, 21-25 patients per day.
• Works with other clinical staff to establish clinical protocols and measures.
• Review mid-level patient medical records as per policy.
• Follows established agency policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
• As appropriate to the position, participates in specified health promotion, education and/or prevention programs.
• Establish, strengthen, and negotiate relationships between the health center and other clinicians, provider organizations and providers in this marketplace.
• Maintains appropriate licensure and certification including meeting and/or exceeding CME requirements.
• Attends all staff and provider meetings as requested.
• Performs miscellaneous job-related duties as assigned.
• Supports the mission of PATHS.
