Job summary

PATHS Community Center in Danville, VA seeks a physician to provide primary care across all ages within a Patient Centered Medical Home model. You will lead a care team, supervise midlevel providers, and ensure evidence-based practice while maintaining high-quality patient care.

The role requires an MD/DO, Virginia license, and DEA certification with CPR; Board certification is preferred. Strong documentation and collaboration with clinical staff are essential.

Responsibilities

• Provides and manages direct patient care, including examinations, diagnoses and treatment.

• Delivers patient education on disease management and prevention.
• Participates in on-call schedule and maintains patient confidentiality.
• Supervises midlevel providers and reviews charts for quality of care.
• Establishes clinical protocols with other staff.
• Maintains licensure and CME compliance.
• Attends staff and provider meetings as requested.

Job Description

SUMMARY

Under the general supervision of the Chief Medical Officer, provides primary care to patients ranging in age from birth to geriatric. Works as part of a care team that practices evidenced based medicine in a Patient Centered Medical Home model of care. Provides clinical leadership and supervision of midlevel providers

Education and/or experience

• Graduation from an accredited school of medicine
• Hold current license to practice in state of Virginia
• Current DEA certificate
• Board Certification preferred
• CPR Certified
• Credentialed per Community Health Center Policies and Procedures for billing services under Medicare/Medicaid
• Supervises assigned midlevel providers in the conduct of clinical care and delegates’ authority when and where needed.
• Reviews assigned midlevel provider’s patient charts to check for quality of care.

Clinical

• Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Obtains and documents, or causes to be obtained and documented; appropriate chief complaint, past medical, family and social history, review of systems, examination, medications and allergies, assessment and plan for each patient within the electronic medical record.
• Provides medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals within electronic medical record as per policy.
• Reviews ancillary test results and coordinates notification to the patient as per policy.
• Provides education in disease management and prevention.
• Completion of appropriate chart documentation within EHR to comply with insurance and reimbursement guidelines
• Completion of all required documentation within EHR within a timely manner as per policy.
• Participates in on-call schedule.
• Maintains patient confidentiality at all times.
• Will see on average, 21-25 patients per day.
• Works with other clinical staff to establish clinical protocols and measures.
• Review mid-level patient medical records as per policy.
• Follows established agency policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
• As appropriate to the position, participates in specified health promotion, education and/or prevention programs.
• Establish, strengthen, and negotiate relationships between the health center and other clinicians, provider organizations and providers in this marketplace.
• Maintains appropriate licensure and certification including meeting and/or exceeding CME requirements.
• Attends all staff and provider meetings as requested.
• Performs miscellaneous job-related duties as assigned.
• Supports the mission of PATHS.
