Job summary

Northern Rivers Family of Services is seeking a Community Service Provider to deliver home and community-based services for children with social, emotional, developmental, health and/or behavioral healthcare needs. The role involves travel to local communities to provide peer and caregiver support and advocacy, reporting to a Community Service Provider Supervisor and working onsite when possible.

Qualifications

• Must demonstrate lived experience as a parent or primary caregiver for children with behavioral or emotional healthcare needs.
• Requires willingness to travel locally to work with families in their communities.
• Valid NYS Driver's License required.
• Documentation and reporting duties will be completed as part of the role.

Responsibilities

• Provide services within a Family-driven Care framework, respecting family choice and voice.
• Work with family members to identify goals and objectives of treatment.
• Provide services based on the needs identified in the Treatment Plan.
• Coordinate with the child and caregiver on scheduling and provision of services.
• Participate in treatment meetings and required trainings.
• Complete all documentation.
