Job Title: Family Partner

Work Location: Westfield, MA

Job Summary:

Duties and Responsibilities:

  • Collaborate with Clinician to complete the CANS Assessment and develop a Family Support Plan with family members experiencing CRA-related issues; arrange referrals, service/support planning, and follow-up.
  • Develop and implement Family Support Plans based on needs identified from intake, screening, and assessment; include specific tasks and responsibilities for family members, the FRC, and other entities approved by the family.
  • Follow-up with families within two weeks of referral to ensure connection with services; within 60 days, assess family satisfaction with services using a Family Satisfaction Survey.
  • Review Family Support Plan with families at six months post-assessment; evaluate adherence and adjust the plan as needed.
  • Facilitate family, youth, and group counseling, as well as psycho-educational groups; support family members who have experienced trauma.
  • Collaborate with community-based service providers and make appropriate referrals.
  • Maintain confidential Family Support Plans and monitor participant records for completeness and compliance, including data input into Gandara’s EMR and UMASS systems.
  • Provide culturally sensitive training and educational information on clinical topics as needed.
  • Adhere to policies and notify the Program Director in case of crises or incidents.
  • Represent the FRC in community and state-wide networking and educational conferences; attend meetings and trainings.
  • Perform other duties as assigned by the SFRC Program Manager and Director.

Minimum Qualifications:

  • Lived experience or familiarity with CRA-related issues
  • Experience working with diverse populations using a family-centered, strength-based approach
  • Availability for weekend and evening hours
  • Valid driver’s license and clearance for driving history and criminal background checks
  • Exceptional interpersonal skills
  • Proficiency in Microsoft Office and related office technology
