Job Description:

Schedule:

40 hours/week, Weekends and holidays as needed

Must hold 4 of the following licenses: Florida (FL), Massachusetts (MA), Ohio (OH), and Maryland (MD).

Position Summary:

  • The Telehealth Nurse Practitioner delivers patient care services through a remote technology platform. You will collaborate with a dedicated team and independently provide holistic, evidence-based care, including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients aged 18 months and above. Encounters are documented in an electronic health record (EHR). Telehealth providers report directly to the Enterprise Initiative Lead.

Responsibilities:

1. Patient-Centered Quality and Safety

  • Evaluate primary care, acute, chronically ill, and transitional care patients, providing healthcare education, counseling, and disposition planning for patients aged 18 months and above.
  • Provide patient counseling on pregnancy prevention, STI prevention, contraceptive care, and medication management.
  • Educate patients on health maintenance and respond to inquiries.
  • Document all patient care within an EHR according to policies and procedures.
  • Coordinate care with internal and external colleagues, including the broader patient-centered medical home.
  • Work effectively within a patient care team, including Providers, Physicians, paraprofessionals, Pharmacists, and other healthcare team members.
  • Increase patient engagement and satisfaction through feedback integration.
  • Create a warm, welcoming environment focused on the patient.
  • Tailor communication to influence quality outcomes and meet patient needs.
  • Collaborate with pharmacy and front office staff to enhance patient experience.
  • Maintain patient confidentiality per PHI and HIPAA standards.
  • Foster positive relationships among practice staff, colleagues, and external partners.
  • Reprioritize tasks throughout the day to meet patient and business needs.
  • Support the overall patient experience by managing clinical and non-clinical duties and patient expectations.
  • Adopt new service offerings and care models quickly.
  • Adhere to CLIENT Health's core values in all interactions.
  • Assist in hiring, developing, and evaluating practice staff.
  • Perform clinic operational tasks such as inventory management, lab follow-ups, phone handling, and maintaining cleanliness.

4. Business Acumen

  • Manage business needs, including budgeting, payroll, inventory, billing, and collections.
  • Implement practice business plans to meet goals.
  • Ensure accurate payment collection through proper documentation and verification.
  • Handle revenue cycle management tasks, including collecting payments and preparing deposits.

  • Work independently, prioritize, solve problems, and advocate for patients and practice.
  • Participate in professional development activities.
  • Maintain professionalism and self-awareness impacting the practice and healthcare industry.
  • Complete continuing education as required to maintain employment.

Experience:

  • Minimum two years of relevant medical experience.
  • High-speed internet with minimum 25 Mbps download and 3 Mbps upload speeds.
  • Unrestricted license in two of the specified states, with ability to obtain multi-state licensure.
  • Effective communication skills.
  • Strong organizational skills, multitasking, initiative, problem-solving, and adaptability.
  • Ability to work autonomously without direct supervision.
  • Proficiency with healthcare information management and technology.
  • Ability to collaborate with colleagues to ensure quality care.

Education:

Master’s degree in Family Nurse Practitioner program, current National Board Certification, and valid State license to practice as an Advanced Practice Nurse.
