Job summary

The Edward M. Kennedy Community Health Center, Inc. seeks a Family Nurse Practitioner to provide comprehensive primary care in an outpatient setting, coordinating with a Board-Certified Physician and collaborating with other departments in patient care.

Active MA NP license required; 3+ years in primary care is preferred. Responsibilities include preventive care, health education, EHR documentation and HIPAA compliance, with a team-based approach across our community sites.

Qualifications

• Active, Unrestricted Nurse Practitioner License in Massachusetts.
• 3+ years of experience as a Nurse Practitioner in a Primary Care setting (a plus).

Responsibilities

• Delivers direct primary care and preventative services to patients across age groups.
• Develops plans of care for prevention, management and maintenance of health.
• Provides health education and counseling to patients and families.
• Cares for acute and chronic illness and urgent care needs.
• Collaborates with multidisciplinary teams and mentors other clinicians.
