Job summary

Community Health Centers of Lane County is seeking a Family Nurse Practitioner to join an interdisciplinary team delivering comprehensive primary care in a Patient Centered Medical Home setting at the Charnelton Clinic.

The NP will diagnose and treat a wide range of conditions, provide education and referrals, and document medical records in compliance with policy; bilingual compensation may apply after testing; travel between clinics during onboarding is expected.

Qualifications

• Master's degree in Nursing and completion of a Nurse Practitioner program for the applicable specialty.
• Oregon RN license in good standing and NP certification in the state.
• Credentialing documents including NPI, DEA, and LA license requirements.
• Bilingual exam may qualify for additional compensation.

Responsibilities

• Provide comprehensive primary medical care in a Patient Centered Medical Home model.
• Diagnose and treat a wide range of health conditions for diverse patients.
• Educate patients and arrange referrals as appropriate; maintain medical records.
• Work within an interdisciplinary care team to support patient health goals.
