Seeking an experienced Family Medicine or Primary Care Nurse Practitioner for an outpatient employment opportunity in Shelton, CT.
The FNP will provide high-quality primary care in an office-based setting, including examining, obtaining medical history, and treating acute and chronic illnesses. They will order, perform, and interpret diagnostic tests.
The role involves creating treatment plans, involving patients and family members, initiating appropriate specialty referrals, and collaborating on care delivery improvements with management, physicians, and staff.
The FNP will work with the care team to close care gaps, improve quality measures, and participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
Responsibilities include working with practice management to ensure compliance with billing and coding requirements.
The position is full-time, Monday through Friday, requiring a CT license, DEA registration, and a minimum of 2 years of practice experience.
