Overview

Position Summary

The Family Nurse Practitioner (FNP) provides comprehensive primary healthcare services to patients across the lifespan. Working with physicians and other healthcare professionals, the FNP evaluates, diagnoses, treats, and manages acute and chronic medical conditions, emphasizing preventive care, patient education, and evidence-based practice. The FNP practices within state and federal regulations, organizational policies, and national standards of care.

Responsibilities
  • Perform comprehensive history and physical examinations.
  • Assess, diagnose, and manage acute and chronic illnesses.
  • Order, perform, and interpret diagnostic tests including laboratory studies and imaging.
  • Develop and implement individualized treatment plans.
  • Prescribe medications and therapies in accordance with state regulations and collaborative practice agreements, when applicable.
Clinical Focus
  • Manage chronic diseases including hypertension, diabetes, hyperlipidemia, COPD, asthma, heart failure, thyroid disorders, and obesity.
  • Provide preventive health services including annual wellness visits, routine physical examinations, immunizations, cancer screenings, health risk assessments, and routine office procedures.
  • Counsel patients on lifestyle modification, nutrition, exercise, smoking cessation, and disease prevention.
  • Coordinate referrals to specialists and ancillary services; review laboratory and diagnostic results and communicate findings to patients.
  • Maintain accurate, timely electronic medical records and participate in quality improvement initiatives and population health programs.
  • Collaborate with physicians, specialists, nursing staff, care coordinators, social workers, and other team members; respond to patient messages and prescription refill requests; participate in clinical meetings and continuing education.
  • Comply with HIPAA, OSHA, Joint Commission, CMS, and organizational policies.
Clinical Responsibilities
  • Primary care management; acute sick visits; chronic disease management; preventive medicine.
  • Women's health (within scope) and men's health; geriatric care; pediatric care (as appropriate).
  • Medication management; health maintenance; patient education; care coordination.
Qualifications
Education
  • Master's or Doctorate degree from an accredited Family Nurse Practitioner program.
Licensure
  • Current unrestricted Registered Nurse (RN) license.
  • Current Advanced Practice Registered Nurse (APRN) license.
  • Current Family Nurse Practitioner (FNP-BC or FNP-C) certification.
  • Current DEA registration (or eligible).
  • Illinois Controlled Substance License (if applicable).
  • Current BLS certification.
  • ACLS preferred.
Experience
  • 1–3 years of Family Nurse Practitioner experience preferred.
  • Experience in primary care, family medicine, internal medicine, community health, urgent care, FQHC, value-based care; new graduates may be considered.
Knowledge, Skills, and Abilities
  • Strong clinical assessment and diagnostic skills; ability to formulate evidence-based treatment plans.
  • Excellent communication and interpersonal skills; strong organizational and time-management abilities.
  • Proficiency with electronic health records (EHR), preferably Cerner; ability to work independently and collaboratively.
  • Commitment to patient-centered care; knowledge of CMS quality measures and preventive care guidelines.
  • Cultural competence and sensitivity to diverse patient populations.
Physical Requirements
  • Ability to stand and walk for extended periods; lift up to 25 pounds occasionally; perform repetitive hand movements for documentation; respond to medical emergencies.
Working Conditions
  • Outpatient medical office or ambulatory clinic; exposure to infectious diseases and bodily fluids.
  • Frequent interaction with patients, families, and healthcare professionals; standard clinic hours with occasional evening or weekend coverage as needed.

Reports To

Medical Director, Director of Primary Care, or Supervising Physician (per organizational structure).
