Ideal candidates hold a BSN, MSN, NY RN/NP licenses, BLS, and board certification, with 8+ years of NP experience. The position emphasizes patient-centered care, collaboration across teams, and opportunities for leadership and growth.

Responsabilidades

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
