Sandhills Medical Foundation, Inc. invites a dedicated Family Nurse Practitioner (FNP) to join our community health team in Sumter, SC.

You’ll deliver comprehensive primary care across the lifespan in a Federally Qualified Health Center setting, emphasizing patient-centered care and teamwork. Responsibilities include diagnosing illnesses, treating acute conditions, and coordinating care with specialists while maintaining HIPAA-compliant EMR documentation.

Qualifications

• Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited program.
• Board certified Family Nurse Practitioner (FNP) with an active, unrestricted license.
• 1–2 years of experience in family practice, internal medicine, or community health preferred.
• Proficient with EMR charting, excellent customer service, and team-oriented clinical skills.

Responsibilities

• Diagnose and treat acute illnesses, injuries, and manage chronic conditions.
• Provide comprehensive primary healthcare and wellness exams within established guidelines.
• Order and interpret laboratory studies and diagnostic testing.
• Prescribe therapeutic treatment plans and medications.
• Coordinate care with specialists, testing facilities, and community health teams.
• Maintain accurate, HIPAA-compliant patient charts within the EMR.
