Job summary

Revere Staffing Partners seeks a compassionate Family Nurse Practitioner to join its multidisciplinary healthcare team in a mission-driven nonprofit FQHC. The NP will provide comprehensive primary care with emphasis on preventive medicine, chronic disease management, and patient education.

The role requires a Master’s or Doctoral NP degree, valid NP certification, and California RN requirements, plus DEA and BLS credentials and at least two years of primary care experience.

Qualifications

• Minimum two years of primary care clinical experience.
• Active, unrestricted DEA registration.
• Current BLS certification.

Responsibilities

• Provide comprehensive primary care, including assessments, screenings, diagnoses, treatment, and preventive services.
• Develop individualized treatment plans after physical exams and histories.
• Prescribe and manage medications per laws and guidelines.
• Monitor progress and adjust care plans as needed.
• Educate patients on prevention, adherence, and healthy lifestyles.
• Collaborate with physicians and behavioral health professionals as part of a care team.
• Maintain accurate documentation in the EHR and coordinate referrals.

Revere Staffing Partners is seeking a compassionate and qualified full-time Family Nurse Practitioner to join its multidisciplinary healthcare team. The Nurse Practitioner will provide comprehensive primary care services, emphasizing preventive medicine, chronic disease management, patient education, and coordinated care.

Key Responsibilities

• Provide comprehensive primary care, including patient assessments, screenings, diagnoses, treatment, and preventive healthcare services.
• Conduct physical examinations, obtain medical histories, and develop individualized treatment plans.
• Prescribe and manage medications in accordance with applicable laws, clinical guidelines, and established protocols.
• Monitor patient progress and treatment outcomes, adjusting care plans as clinically appropriate.
• Educate patients about disease prevention, medication adherence, healthy lifestyle choices, and overall wellness.
• Collaborate with physicians, behavioral health professionals, and other members of the interdisciplinary care team.
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Coordinate referrals, specialist consultations, and follow-up care to ensure continuity of treatment.
• Participate in team meetings, clinical cross-coverage, and after-hours on-call rotations as assigned.
• Contribute to the delivery of accessible, patient-centered, culturally responsive healthcare for diverse communities.

Required Qualifications

• Master’s degree or doctoral degree from an accredited Nurse Practitioner program.
• Appropriate national board certification and compliance with California Board of Registered Nursing requirements.
• Active, unrestricted Drug Enforcement Administration (DEA) registration.
• Current Basic Life Support (BLS) certification.
• At least two years of primary care clinical experience.

• Experience working in a community health center, Federally Qualified Health Center (FQHC), or other healthcare setting serving underserved populations.
• Strong communication, clinical assessment, and decision-making skills.

Compensation and Benefits

Offers a competitive salary and benefits package, including:

• Annual salary of $158,000–$185,000.
