LRSolutions, LLC is seeking a Family Nurse Practitioner for outpatient primary care roles in New York. Flexible hours, Monday–Friday, with no on-call requirements, and a strong emphasis on patient-centered care and team collaboration.

The ideal candidate will have a BSN and MSN, NY RN/NP licensure, and at least 8 years of NP experience, delivering preventive, acute, and chronic care in a collaborative, HIPAA-compliant environment.

Qualifications

• BSN and MSN from an accredited nursing school.
• NY RN & NP license, BLS, Board Certified.
• 8+ years experience as a NP.
• Strong diagnostic, communication, and interpersonal skills.

• Entrepreneurial mindset with ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.

A well-established, physician-led group with locations throughout Long Island, is currently seeking to hire a Family Nurse Practitioner. This practice has built a stellar reputation for clinical excellence, patient satisfaction, and multidisciplinary collaboration. 100% outpatient.

Key Responsibilities:
