Job summary

Top Echelon Software LLC is seeking a Family Nurse Practitioner to join our physician-led practice on Long Island. The role focuses on 100% outpatient primary care, providing preventive, acute, and chronic disease management in a flexible schedule.

You will collaborate with physicians and care teams, oversee staff, and ensure HIPAA and payer guidelines compliance. A strong diagnostic skillset and patient-centered approach are essential for building a thriving local practice.

Qualifications

• BSN and MSN from an accredited nursing school or equivalent.
• NYS RN & NP license, BLS, Board Certified.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.
• 8+ years experience as a NP.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.

Job Type: Full time or Pat time. Flexible hours, Monday–Friday, No call.

Key Responsibilities:
