Qualifications

Responsibilities

Overview

We are seeking a dedicated and compassionate Family Nurse Practitioner to provide high-quality, comprehensive primary care to patients across the lifespan. This role focuses on the diagnosis and management of common and chronic conditions, with a strong emphasis on health promotion, disease prevention, patient education, and collaborative, team-based care. The Family Nurse Practitioner will work closely with the Medical Director and interdisciplinary care teams to support quality outcomes and continuous improvement in clinical practice.

Key Responsibilities

• Provide comprehensive primary care services, including health assessments, physical examinations, diagnosis, and treatment planning.
• Order, interpret, and evaluate diagnostic tests and develop individualized plans of care.
• Prescribe medications and treatments in accordance with scope of practice and regulatory requirements.
• Deliver preventive care, health screenings, counseling, and patient/family education.
• Collaborate with physicians, nurses, and other healthcare professionals; make referrals to specialty and community resources as appropriate.
• Manage and document patient care accurately and in a timely manner.
• Provide age-appropriate care to pediatric, adolescent, adult, and geriatric patient populations.
• Participate in quality improvement initiatives, committees, and special projects as assigned.
• Provide telephone coverage, home or hospital visits, and on-call support as directed.
• Serve as a preceptor, mentor, and role model for students and staff when applicable.
• Maintain compliance with organizational policies, confidentiality standards, and regulatory requirements.

Qualifications

Education

• Bachelor’s and Master’s degree in Nursing.
• Graduate of an accredited Nurse Practitioner program.
• Bilingual (Spanish preferred).

Experience & Skills

• Strong clinical experience as a Family Nurse Practitioner.
• Excellent clinical judgment, problem-solving, and leadership skills.
• Effective organizational, communication, and interpersonal abilities.
• Basic computer proficiency.

Licensure & Certifications

• Current New York State Registered Nurse License and Registration.
