Job summary

Community Health Centers of South Central Texas, Inc. is seeking a Family Nurse Practitioner to join our Luling Community Health Center and Lockhart Family Practice Center locations. This is a full-time position with travel required.

You will conduct comprehensive assessments, diagnose illnesses, collaborate with a multidisciplinary team to develop and implement patient care plans, and educate patients and families on preventive care and healthy lifestyle choices.

Qualifications

• Must hold current Nurse Practitioner certification.
• Must be licensed as a Nurse Practitioner in Texas.
• Primary care nursing experience is required.
• Excellent communication and interpersonal skills.
