BAS HealthCare is seeking an Advanced Practice - Family Nurse Practitioner to provide comprehensive family practice care for a mixed pediatric and adult patient population in Fortuna, CA. The role involves about 18 patient encounters per shift, Monday to Friday, 8:00 AM to 5:00 PM, with chart closure required within 72 hours.

Candidates must hold an active California license, and have a DEA license before start.

Job Details:

• Provide comprehensive family practice care to a mixed pediatric and adult patient population
• 18 patient encounters per shift, Monday through Friday, 8:00 AM - 5:00 PM
• Chart closure required within 72 hours
• Must have a minimum of 3 years of experience in a similar clinical setting
• Active California license required; licensed in CA only
• Active DEA required prior to start date
• PAs must have a CA Controlled Substance Education Course Certificate
• NPs must have a Furnishing License with Schedule II
• BLS certification required
• Providers must be ORP (order, referring, and prescribing) enrolled in MediCal or willing to enroll during credentialing
• Candidate CVs must include month and date on all work experience with all gaps in employment accounted for
• Required immunizations prior to start date: Flu Vaccine, TB Status/X-Ray Results (within 90 days of start), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, and Tdap Immunization (within the last 10 years)
• Credentialing timeframe is approximately 45-90 days
• 30-day notice required for ending all assignments
