Job summary

Team Select Home Care is seeking an Office Administrator to coordinate referral, intake, and onboarding activities under the Director of Operations. You will work with local operations and clinical teams to ensure smooth start-of-care and maintain high levels of customer service.

The role supports recruitment, HRIS tasks, and collaboration with Community Relations Managers to meet market goals. Strong organizational and communication skills are essential for success in this position.

Qualifications

• Bachelor degree preferred in business-related field.
• Minimum 1 year experience in healthcare/home health preferred.
• Strong communication and organizational skills required.

Responsibilities

• Coordinate intake, referral, and onboarding processes under DOO.
• Maintain customer service and accurate communication with patients and families.
• Support recruitment activities and HRIS-related tasks.
• Assist CRM in achieving market goals and events.

Job description

The Family Navigator (FN) is responsible for coordinating the referral, intake, and onboarding process in an office of the Long-Term Home Health Division. In this role, you will report to the Director of Operations (DOO).

Duties/Responsibilities

Ensures under the DOO that the needs of the agency, referral sources, and all patients/families going through the intake process are met through proper communication, customer service, and oversight Works closely with the local Operations and Clinical teams to coordinate efficient, appropriate intake and start of care processes Drives on-going patient and employee retention through customer service and communication Assists with all recruitment functions for the office; proficient in HRIS systems, and all other facets of recruitment role Assists DOO in achieving financial metric goals Assists Community Relations Managers (CRM) in achieving market sales goals including in-office and out-of-office tasks and events Coordinates non-clinical functions of the referral, intake, and onboarding process in conjunction with Operational and HR staff to ensure all agency requirements are met Collaborates with HR staff to drive efficient hiring processes for new staff Maintains a current awareness of the abilities and limitations of RN Case Management staff in collaboration with Clinical Supervisors and DOO Demonstrates knowledge and adherence to the policies and procedures of company, and holds staff accountable for carrying out the duties outlined in the manuals, state and federal regulations and company's Best Practices and processes Demonstrates an ability to identify and solve problems with initiative and good judgment to reach quality decisions Maintains rapport with clients and employees and effectively promotes harmonious interpersonal relationships Meets all deadlines on special projects as assigned Maintains confidentiality of all employee, patient/client PHI and company issues Adheres to and promotes the Compliance Plan, policies and procedures Ability to maintain operational caseload as needed On-call responsibility as required by DOO Attends in services as requested Performs all other job duties as assigned

Pay Range: $65,000 - $80,000 / salary with bonus
