Job summary

Mid-South Senior Care is seeking a Family Model Provider in Tennessee to offer housing, supervision and personal care in a family home setting. Responsibilities include daily living assistance, transport to appointments, and community activities.

Caregiving experience is required, candidates should be 21+, own or rent a home in Tennessee, and have a valid TN driver's license with insurance. CPR/First Aid certification and background checks are required or obtainable.

Qualifications

• Caregiving experience required.
• Must be 21 years or older; read/write in English.
• TN driver’s license with auto insurance.
• Provide or obtain required trainings and certifications.

Responsibilities

• Provide housing, supervision and personal care in a home setting.
• Assist with daily living activities and meal preparation.
• Transport to medical appointments and community activities.
• Medication administration if certified and required.
