Job description

Seeking a Family Medicine Physician who ispassionate about whole-person care and committed to delivering evidence-basedprimary care services. As a provider, you will work with a team of healthcareprofessionals to deliver practical treatment for chronic and acute care needs,disease management services, and preventive care.

Responsibilities :

  • Provide primary care to 10-15 patients per day,focusing on prevention, practical treatment for chronic and acute care needs,and providing disease and case management services.
  • Develop biopsychosocial patient care plans inpartnership with the patient and your care team, including an onsite WholeHealth Coach, that meet the needs of each unique patient.
  • Collect comprehensive health history, performappropriate physical examinations, conduct diagnostic testing, and prescribemedications with the convenience of onsite lab and pharmacy.
  • Assess patient population needs and driveparticipation in annual whole health evaluations, health coaching, and otherhealth promotion activities.
  • Provide clinical leadership, participate in teammeetings, and support change management.
  • Ensure that all customer contractual needs andexpectations are met in partnership with your care team.
  • Rotate with other providers in the region toensure coverage after-hours.
  • Participate in organizational activities andcommittees to optimize processes and programs.
  • Achieve target objectives by improving thehealth status of the eligible population and meeting established benchmarks forclinical outcomes, engagement, and satisfaction.

Qualifications :

  • MD or DO degree with completion of a FamilyMedicine or Internal Medicine Residency, Family Medicine preferred.
  • Current Board Certification by ABMS (ABFM / ABIM)or AOA.
  • 3+ years of primary care experience.
  • Current, valid, and unrestricted medical licenseand valid DEA number.
  • Team-based care experience.
  • Experience working in EMR systems, NextGenpreferred.
  • Experience in coding and documentation,including Medicare Advantage and HCC.
  • Experience with Google Workspace.
