Job summary

MyCHN (Community Health Network) is a nonprofit FQHC serving Greater Houston. We are seeking a Family Medicine Physician to provide comprehensive primary care within our Patient-Centered Medical Home model, delivering high-quality, evidence-based care to patients of all ages.

You will collaborate with multidisciplinary teams, manage acute and chronic conditions, and promote wellness and preventive care while supporting underserved communities.

Qualifications

• Doctor of Medicine degree required.
• Current and unrestricted Texas Medical Board license.
• Board certification or board eligibility in Family Medicine.
• Strong clinical assessment, diagnostic, and treatment skills.
• Commitment to patient-centered, whole-person care.
• Ability to work with diverse socioeconomic backgrounds.
• Strong communication and interpersonal skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Passion for improving access to quality healthcare within the community.

Responsibilities

• Provide comprehensive primary care services to patients of all ages.
• Evaluate, diagnose, and manage acute and chronic health conditions.
• Obtain and assess patient health histories through interviews, examinations, and diagnostic testing.
• Identify and address risk factors through clinical assessment and evidence-based care.
• Develop and implement individualized treatment plans.
• Promote wellness, preventive care, and health maintenance strategies.
• Provide patient education and counseling to support healthy lifestyles and disease management.
• Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care.
• Refer patients for specialty care and additional services as appropriate.
• Maintain accurate, timely, and comprehensive patient documentation.
• Ensure prompt follow-up on patient calls, laboratory results, referrals, and prescription needs.
• Support quality improvement initiatives and achievement of healthcare outcome measures.
• Maintain professional licensure and continuing education requirements.
• Perform other duties as assigned.
