Key Responsibilities

• Collaborate with a multidisciplinary team of healthcare providers.
• Provide comprehensive care focusing on physical, mental, and social health at each visit.
• Manage a patient panel primarily consisting of geriatrics with Medicare Advantage coverage.
• Achieve quality metrics with bonus incentives.
