Position Overview

The Family Practitioner is a skilled member of the health team who provides medical and educational services within the scope of their professional license in Indiana. This role serves offices in Gary, Hebron, and Portage. Candidates with supervisory experience or interest in future leadership roles as Medical Directors are encouraged to apply.

Organization Details

Porter-Starke Services, Inc., based in Valparaiso, is a Community Mental Health Center serving northwest Indiana, with additional offices in Portage, Knox, and LaPorte. It also operates Marram Health Center, a Federally Qualified Health Center participating in the NHSC/HRSA Loan Repayment program. Open to J-1/H1-B visa sponsorship candidates.

Primary Responsibilities
  • Perform physical assessments using observation, inspection, auscultation, palpation, and percussion.
  • Follow standardized protocols and guidelines to treat patients.
  • Obtain and evaluate medical histories, provide counseling, and educate families on health conditions.
  • Promote high standards of medical practice.
  • Educate patients and families on preventative care and treatments.
  • Collaborate with patients, families, and community to set health goals.
  • Develop respectful, collaborative relationships with patients and families.
  • Ensure patients receive proper education on continued care.
  • Work with the care team to provide patient-centered care.
  • Maintain accurate, confidential medical records.
  • Document evaluations, diagnoses, treatments, outcomes, education, referrals, and consultations in compliance with standards.
  • Meet quality improvement standards for record-keeping.
  • Maintain a safe, infection-free environment.
  • Keep professional licenses and certifications current.
  • Prescribe medications as needed.
  • Perform other duties as assigned.
Minimum Qualifications
  • Current, unrestricted Family Practitioner license in Indiana or ability to obtain it.
Qualifications

  • Experience or training in primary care required.
Salary

DOE, range $220,000 - $275,000
