Job summary

Fairview Health Services seeks a Family Medicine physician to join the M Health Fairview Clinics - Elk River in Elk River, Minnesota. You will provide comprehensive primary care for families in a collaborative, team-based setting with 34 patient contact hours per week.

Expect a 1.0 FTE schedule with 15-18 patient encounters daily, and a supportive environment within an Accountable Care Organization. EPIC is the electronic medical record, and Nabla AI assists with documentation.

Qualifications

• Graduate of an accredited ACGME/AOA Family Medicine residency program.
• Board Certified or Board Eligible (ABMS or AOA).
• Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
• Ability to obtain and maintain DEA certification.
• Meet credentialing and privileging requirements.

Responsibilities

• Provide comprehensive family medicine care within a 15-provider primary care practice (10 Physicians, 5 Advanced Practice Providers).
• Collaborate within a team-based model focused on quality outcomes, care coordination, and population health management.
