Responsibilities

• Obtain health history and perform physical examinations utilizing medical instruments and equipment.
• Order or perform tests, analyses, and diagnostic imaging to inform diagnosis.
• Analyze test results and diagnose patient conditions.
• Diagnose and treat acute and chronic diseases and injuries.
• Administer or prescribe treatments and medications per diagnosis.
• Prescribe medications per state and federal guidelines.
• Promote health with diet, hygiene, and disease prevention advice.
• Prepare records and reports per established procedures.
• Counsel patients and families on diagnosis and treatment plans.
• Participate in peer review, quality assurance, and provider meetings.
• Make referrals to specialists when appropriate.
• Support HMFP culture and promote team-based delivery of patient care.
