Job summary

Proforma Health Inc. is seeking a Family Medicine Physician for full-time, outpatient care in Stayton, OR. You will provide comprehensive primary care, collaborating with APPs and clinical staff in a team-based setting, with a Monday–Friday schedule and supportive work-life balance.

Qualifications include board certification or eligibility in Family Medicine; new graduates are encouraged to apply. Visa sponsorship is preferred, with strong emphasis on communication and teamwork.

Role: Family Medicine Physician

Responsibilities: Provide 100% outpatient primary care with a focus on patient relationships and clinical excellence. Manage approximately 18-20 patients daily in a collaborative, team-based environment supported by APPs and clinical staff. Handle minimal phone-only call, with optional neonatal call, supported by nurse triage. Maintain a Monday–Friday schedule, ensuring work-life balance.

Compensation & Benefits: $325K base salary plus production incentives, $25K sign-on bonus, relocation assistance, and 34 days of PTO in the first year. Benefits include CME support, licensure assistance, and comprehensive health coverage.
