You will deliver coordinated in-person and virtual care using our technology platform and EHR, collaborating with clinical, operations, and technology teams to continuously improve care delivery and patient outcomes.

Qualifications

• Family Medicine training and ability to manage a broad range of primary care needs.
• Strong clinical judgment in preventive and acute settings.
• Experience delivering care in both in-person and virtual workflows.
• Proficiency with EHR/clinical technology and coordinated, quality-focused care.
