Job summary

HealthPartners Dassel Clinic in Dassel, MN is seeking a Physician Assistant or Nurse Practitioner to join our rural primary care practice on a part-time basis (3 days a week). The clinic serves a loyal patient population an hour west of Minneapolis and is part of HealthPartners and Hutchinson Health.

The successful candidate will provide care across all ages, perform exams, order labs and imaging, educate patients, and collaborate with local health services.

Qualifications

• Degree from a recognized school.
• Physician Assistant or Nurse Practitioner certification.
• Experience in the ongoing care of adults and children in a rural clinic is desirable.
• Experience in Women's Health is required.

Responsibilities

• Ensures the plan of care for the neonate, pediatric, adolescent, adult and geriatric patient populations reflect a broad range of ages and needs.
• Provides scheduled primary care efficiently and accurately.
• Provides ongoing patient education and counseling.
• Treats acute care problems efficiently and accurately resulting in patient satisfaction.
• Provides timely chart documentation that is accurate and thorough.
• Orders labs, x-rays, and diagnostic tests to facilitate the correct diagnosis in a cost-efficient manner.
• Determines when a patient's problem requires physician input and facilitates discussion with a physician.
• Participates in community health programs promoting wellness and health education.
• Works with Hutchinson Health’s occupational health services to promote health and safety in local business and industry.
• Participates in an active public relations program which promotes Hutchinson Health.
• Performs all other related duties as assigned.

Hutchinson Health is seeking a Physician Assistant or Nurse Practitioner to join our primary care practice in Dassel MN. Dassel Clinic is a well-established rural health clinic with a loyal patient population. One hour west of Minneapolis, the Dassel Clinic draws from well-established communities.

Essential Duties & Responsibilities:

1. Ensures the plan of care for the neonate, pediatric, adolescent, adult and geriatric patient populations reflect:
   1. an understanding and ability to obtain information/perform assessment and react effectively to the unique needs of these patients;
   2. assessment information that reflects physical, psychosocial, developmental and cognitive data specific to the age of the patient; and an understanding of the range of treatment needed by these patients.
2. Provides scheduled primary care efficiently and accurately.
3. Provides ongoing patient education and counseling.
4. Treats acute care problems efficiently and accurately resulting in patient satisfaction.
5. Provides timely chart documentation that is accurate and thorough
6. Orders labs, x-rays, and diagnostic tests to facilitate the correct diagnosis in a cost-efficient manner.
7. Determines when a patient’s problem is serious enough to seek advice from a physician and subsequently contacts a physician to discuss patient’s care.
8. Participates in community health programs which promote wellness and health education
9. Works closely with Hutchinson Health’s occupational health services to promote health and safety in local business and industry.
10. Participates in an active public relations program which promotes Hutchinson Health.
11. Performs all other related duties as assigned.

Education, Training or Degree Required:

1. Degree from a recognized school.

License/Registration/Certification Required:

1. Physician Assistant or Nurse Practitioner certification

Experience and Skills:

1. Experience in the ongoing care of adults and children in the setting of a rural clinic is desirable
2. Sound knowledge of diagnosis, evaluation, and management of acute and chronic family medicine. This should include, in part, history taking, physical examination, appropriate use of ancillary lab and radiology testing, and basic treatment plans. Use of consultation with physicians and allied health staff is critical.
3. Experience in Women's Health is required
