Job summary

Ascension Health is seeking a Nurse Practitioner or equivalent for an outpatient Family Medicine role in West Allis, WI. Join an established team of 4 physicians and 1 APP to diagnose, manage, and treat acute and chronic conditions in a supportive setting.

Required Wisconsin NP/RN credentials with a Master’s in Nursing; BLS preferred. This employed position offers a $5,000 start bonus, relocation assistance, CME allowance, and tail malpractice coverage.

Qualifications

• Nurse Practitioner credential from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• Registered Nurse credential from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
• BLS Provider credential is preferred (American Heart Association or American Red Cross).

Responsibilities

• Diagnose, manage, and treat acute and chronic health conditions in an outpatient clinic setting.
• Collaborate with physicians and multidisciplinary team members to ensure coordinated care.
• Maintain detailed, accurate health records in accordance with organizational policies.

Join the team at AMG Gateway, where you will drive the expansion of individualized primary care services in the West Allis community by diagnosing and managing acute and chronic health conditions within a supportive, close-knit team environment.

• Join an established team of 4 physicians and 1 APP

• Call structure: Not required at this time

• Practice setting: Outpatient

• Employed position within a nonprofit health system

• Board Certified or Board Eligible in Family Medicine

• Deliver comprehensive patient care by diagnosing, managing, and treating acute and chronic health conditions to optimize health outcomes for individuals and families in an outpatient clinic setting.

• Drive clinical collaboration by working alongside physicians and multidisciplinary team members to ensure smooth care coordination and continuity across the network.

• Maintain detailed, accurate health records in accordance with specialty standards and organizational policies to ensure patient safety and top-tier quality of care.

Licensure / Certification / Registration

• Required Credential(s):
