Job summary

Ascension Calumet Clinic in Chilton, WI is seeking a Nurse Practitioner or Physician Assistant for full-time outpatient rural primary care. You will provide comprehensive, patient-centered care, including health assessments, histories, testing, diagnosis and treatment of acute and chronic conditions, and preventive services.

Work on campus at Ascension Calumet Hospital with a 1:7 call structure. Benefits include a $5,000 sign-on bonus, a $2,000 CME allowance, RVU incentives, and tail malpractice

Qualifications

• Requires NP or PA licensure in Wisconsin.
• RN license in Wisconsin required.
• BLS Provider required or accepted (American Heart Association or Red Cross).
• DEA credentialed (preferred).

Responsibilities

• Conduct health assessments and patient histories.
• Order and interpret diagnostic tests.
• Diagnose and treat acute and chronic conditions.
• Deliver preventive care services.
• Educate patients and families on options and resources.
• Collaborate with physicians to develop individualized care plans.
• Manage patient panels in a high-volume rural clinic.

How you’ll make an impact in this role

This is an exceptional opportunity for a Nurse Practitioner or Physician Assistant to join a busy, well-established outpatient rural primary care practice with a strong referral base and the full support of a robust health system network, conveniently located on the campus of Ascension Calumet Hospital.

• Join an established team of 1 physician and 5 APP's
• Call structure: 1:7
• Practice in a high-volume rural primary care clinic with a supportive, team-oriented environment within a busy and collaborative setting with an average of 12-15 patients per day
• On campus with Calumet Hospital which was recognized as a 2024 Top 100 Critical Access Hospital by the Chartis Center for Rural Health for outstanding performance among the nation's rural hospitals.
• Provide comprehensive, patient-centered care by conducting health assessments, obtaining patient histories, ordering and interpreting diagnostic tests, diagnosing and treating acute and chronic conditions, and delivering preventive care services.
• Educate and counsel patients and their families on preventive health, treatment options, and community resources while collaborating with physicians to develop, implement, and document individualized care plans.
• Located in the scenic Chilton, WI area. Enjoy small-town charm with easy access to Appleton, Green Bay, and outdoor recreation

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education

• Master's degree of Nursing required.
