Overview

CarleHealth is seeking an additional Advanced Practice Provider to join our Family Medicine Team in Metamora, Illinois.


Opportunity Details:

  • Float position that will be based out of our Metamora clinic but will provide coverage at our different Family Medicine and Walk-In clinics in the Greater Peoria region
  • Work with established teams consisting of Physicians and other experienced APPs
  • Experienced staff, both clinical and non-clinical who know the patients extremely well
  • X-ray and lab services available onsite at most locations
  • Average patients/day: 12-16
  • Fully integrated system with clinics and hospitals on Epic EMR to improve care coordination
  • No call requirements

Education Requirements:

  • Master's degree in Nursing or Physician Assistant
  • Family Practice experience strongly preferred

Licensure/Certification Requirements:

Current Illinois registered nurse and advanced practice nurse/physician assistant licensure.
