Job summary

Carle Health in Mahomet, Illinois is seeking a Family Medicine Advanced Practice Provider to join our primary care team. You will work in an ambulatory setting with a collaborative team, Monday through Friday, 8:00am–5:00pm, and may perform in-office procedures as needed.

The patient population is mostly adults with some pediatric visits, in a supportive community clinic. Applicants should have a Master’s degree in Nursing or Physician Assistant, and Illinois APRN/PA licensure; ideally two years

Responsibilities

• Provide patient care in an ambulatory Family Medicine setting along with a physician, APP, nurse and CMA.
• Maintain a typical M-F 8:00am-5:00pm schedule.
• Possibility to perform in-office procedures as applicable.
• Care for a broad patient population, mostly adults with some pediatric patients.

Education Requirements

Compensation and Benefits

The compensation range for this position is $56.37 - $73.29/hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
