St. Charles Health System is seeking a Family Medicine Advanced Practice Provider (NP/PA-C) in La Pine, OR to deliver comprehensive outpatient care for all ages within a Medical Home model.

The APP will work under supervision with a physician and a dedicated care team, aiming for high-quality outcomes and patient satisfaction. The position requires outpatient Family Medicine experience, relevant licensure, and strong collaboration skills.

Qualificações

• Experience in outpatient Family Medicine and acute care management.
• Proficient in using EHR systems and documenting care efficiently.
• Strong communication with patients, families and care teams.
• Ability to work in a collaborative, team-based environment.

Responsabilidades

• Deliver evidence-based Family Medicine within license and credentialing guidelines.
• Prescribe medications per policies and coordinate with pharmacology staff.
• Document, code and bill for all patient care in the EHR accurately and timely.
• Collaborate with SCMG teams to ensure a world-class patient experience.
• Provide patient and family education on preventive care and treatment plans.
• Participate in clinic after-hours call duties and quality improvement initiatives.
