• Develop service plans and provide referrals/linkages to treatment resources.
• Coordinate linkages and follow-up activities, identify barriers and assist in barrier removal.
• Conduct case management with client Family Safety Supervision Plan and maintain records.
• Provide monthly progress reports to DCFS Family Safety staff."
• Manage discretionary funds for treatment support with proper audit trails.
