Job summary

Saint Francis is seeking a Family Intervention Specialist to deliver Intensive In-Home services using the Family Centered Treatment (FCT) model. You will conduct family and individual sessions in clients’ homes, coordinate care with partners, and ensure fidelity to the FCT framework.

You will build safety plans, monitor progress, and adapt visits to family needs, including evenings and weekends, while maintaining thorough documentation and compliance with SOPs.

Qualifications

• Bachelor’s degree in social work, counseling, psychology required.
• Master’s degree in social work, counseling, psychology or related field preferred.
• At least one year of experience working with children and families, preferred.
• Must pass background and safety clearances.

BASIC PURPOSE OF THE JOB

The Family Intervention Specialist (FIS) provides Intensive In-Home services utilizing the Family Centered Treatment model.

ESSENTIAL FUNCTIONS

Begin in Wheels of Change © at time of hire completing the first 4 units within 7 business days of hire. The first 4 units must be completed prior to serving clients. Training for each phase of FCT must be completed prior to providing services required in each phase.

Complete all requirements to be certified in the FCT model within 9 months of first case assignment.

Participate in team and individual supervision in order to staff cases effectively and create a high level of individual and team accountability.

Utilize the FCT model to provide Intensive In-Home services in Diversions and Reunification cases, adhering to and ensuring fidelity to the FCT model.

Conducts family and individual sessions as needed in clients’ own homes and other settings when convenient for the family, spending an average of 4 hours per week and a minimum of 2 weekly contacts with clients in compliance with expected FCT dosage.

Provides FCT-endorsed experimental interventions unique to each family that promotes the achievement of safety, permanency, and well-being for the children, including evidence-based and trauma-informed in-home services.

Conducts initial and on-going assessments that are strengths based and individualized to be incorporated in treatment plans.

Maintain a flexible schedule to meet family availability (including evenings/weekends as needed).

Implement interventions to ensure any gaps related to the family’s basic needs are filled while helping the family also plan for long term sustainability.

Complete all documentation in a timely manner, including strict adherence to the timeline for FCT fidelity documents, family plan, and completing progress notes within a 48-hour timeframe (2-days, not business days). When possible, complete progress notes and family plans with family in session

Provide youth and families with necessary transportation and help them plan for long term transportation needs.

Process external referrals to meet identified client needs.

Manage scheduling and adjust intensity with individuals and families as needed to ensure consistent service delivery.

Engage all key family members and help families develop a support network in their natural environment.

Deliver interventions designed to develop problem-solving skills in both parents and children.

Collaborate with community partners, schools, courts, and other service providers to coordinate care and support family goals.

Include the family in individualized treatment plans that are periodically updated to reflect the goals of the family and child and focuses on empowerment and increasing parent accountability.

Ongoing monitoring for client safety.

Assess and respond to crisis situations, including safety planning, deescalation coordination with emergency services when necessary [24/7]. Maintain awareness to safety concerns within the home and community.

Serve as the primary point of contact for families experiencing crisis situations during the week, outside of standard working hours. While the first line of communication, consideration for adequate sleep and personal well-being is prioritized.

Focus on client satisfaction throughout service delivery.

Implements Saint Francis’ policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, and local rules, statutes, regulations, and contractual terms. Ensures clients’ rights are protected.

Attend training, support opportunities and/or coaching sessions for ongoing development, as required.

Reports unusual incidents through appropriate Risk Management, clinical, and safety channels.

Ensures clients’ rights are protected. Ensure all clients have received their rights and maintains a signed copy in each file.

Is knowledgeable of and follows all safety procedures.

Qualifications

MINIMUM JOB REQUIREMENTS

Bachelor’s Degree in Social Work, counseling, psychology required

Master’s degree in Social Work, counseling, psychology or related field preferred

At least one year of experience working with children and families, preferred.

Must be 21 years of age.

Must pass a drug screen, DFPS and Child Abuse and Neglect Central Registry Clearance check

FBI fingerprint check.

Lifting Requirements of 50 lbs.

Must have a valid driver’s license, acceptable driving record and reliable transportation.
