Responsibilities

• Provides direct patient care by obtaining history, performing exams, and forming assessment and plan.
• Administers or prescribes treatments and medications.
• Follows up and monitors patient progress throughout treatment plan.
• Documents the evaluation and plan of care.
• Provides patient education on health topics.
• Collaborates with in-house allied health professionals for coordinated care.
• Maintains regular and predictable attendance.
• Performs other duties as assigned.
