Responsibilities include on-call coverage, preventive care, referrals, and strong documentation. Requires CT MD/DO license, DEA, BLS/ACLS, and board certification or eligibility.

Qualifications

• Doctoral degree (MD or DO) and board certification or eligibility in a primary care specialty.
• Strong community health orientation with patient care management experience.

Responsibilities

• Provide patient centered medical care to adults per standards and policies.
• Participate in on-call rotation and coordinate care across primary and behavioral health teams.
• Ensure thorough documentation, referrals, and follow-up on education and self-care.
• Collaborate with departments to meet preventive health and care coordination goals.
