Qualifications

• Graduate of an accredited Medical or Osteopathic School.
• Completion of an ABMS-approved Family Medicine or Internal Medicine Residency.
• Board Certification or Board Eligibility in Family Medicine or Internal Medicine.
• Ability to obtain Minnesota Medical Doctor or Doctor of Osteopathy licensure
• Valid DEA Registration.
