Job summary

Catholic Community Services (FBH Aberdeen site) seeks a Mental Health Care Coordinator to provide intensive, strength-based mental health services to children and families in their homes and community settings. The role includes crisis planning, team coordination, and after-hours support with flexible scheduling.

The position is based at the Aberdeen site, with services delivered across community homes. A Bachelor’s degree in a relevant field, and a valid driver’s license are required; on-call

Qualifications

• Bachelor’s degree or higher in social work, psychology, behavioral sciences or equivalent.
• Experience serving children and/or families in mental health or social services preferred.
• Ability to drive to family homes and communities; valid license and insurance required.

Responsibilities

• Provide intensive and strength-based therapeutic mental health services to children, youth and families in the community.
• Engage with families with compassion, respect and enthusiasm; facilitate crisis planning and safety.
• Coordinate crisis response, ensure safety plan communication, and arrange aftercare supports.
• Meet productivity and documentation standards with strength-based approaches.
• Facilitate placement into foster care when needed and coordinate team plan implementation.
