Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to services. You will collaborate with the treatment team, document progress, and recruit new referrals to maintain caseload.

You will participate in weekly Treatment Team meetings, ensure regulatory and COA standards compliance, and provide 24/7 on-call support as required.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client.
• Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities.

Qualifications

• Bachelor’s degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field, is required.
• Minimum of two years experience working with children and/or families. This experience may include internships and volunteer work.
