Health Connect America is seeking a Family Centered Treatment Specialist to provide intensive in‑home and community‑based behavioral health services to children and families. In this role, you will conduct assessments, develop and implement family‑centered treatment plans, deliver counseling and skill‑building interventions, and coordinate care with schools, agencies, and community partners.

You will document services, monitor progress, and participate in multidisciplinary team meetings.
